Due to receiving another supply of vaccine, we are now opening the clinic up to the following UCFSD students:
-ALL K-12 High Risk Students
-Kindergarten through and including 5th grade Students
Clinic Date/Time: Monday, November 23, 2009, 3pm - 8pm
Location: Unionville High School Cafeteria (Enter via the Gymnasium Entrance)
UCFSD will be hosting an H1N1 vaccine clinic on Monday, November 23rd, 2009. Clinic hours are 3:00PM – 8:00PM. The location of the clinic is the Unionville High School cafeteria. Due to additional vaccine received, we now have vaccine to accommodate our K-12 high risk students and all K-5 elementary students. No registration is necessary. Students in elementary and middle school must be accompanied by a parent / guardian. High school students may be unaccompanied. ALL students need a Consent Form with parent / guardian signature.
The injectable vaccine, manufactured by Novartis will be used for our high risk students and it has been received in multi-dose vials – single dose drawn individually for each injection. The nasal spray vaccine, manufactured by MedImmune will be used for our healthy K-5 students and is a single dose delivery system.
Please, if possible, download the Consent Form, complete and bring to the clinic on Monday. Please also read the appropriate Vaccine Information Statement (VIS) form as it applies to the type of vaccine to be received.
